Dental
The Michigan Conference dental coverage with MetLife gives you access to one of the largest networks of dental providers in the country at a negotiated cost. Click here for more information on the Michigan Conference plan.

MetLife: There are thousands of general dentists and specialists to choose from nationwide — so you are sure to find one who meets your needs. You can receive a list of these participating dentists online at www.metlife.com/dental or call 1-800-942-0854 to have a list faxed or mailed to you. With your PPO Plan, you can visit any dentist but you pay less out-of-pocket when you choose a PPO dentist. Please note: You will not receive an ID card from MetLife. You will simply supply your SSN to your provider and let them know you have MetLife or you can access a copy of the card via the MetLife mobile app. Questions? Click here.

Flexible Spending Accounts (FSA)
A Flexible Spending Account (also known as a flexible spending arrangement) is a special account you put money into that you use to pay for certain out-of-pocket health care costs. Click here for the Michigan Conference claim form.

  • You can use a medical Flexible Spending Account (FSA) to pay for copayments, deductibles, some drugs, and some other health care costs. FSAs are limited to $2,850 per year.
  • You don’t have to pay taxes on this money. This means you’ll save an amount equal to the taxes you would have paid on the money you set aside.
  • You can use funds in your FSA to pay for certain medical, dental, and vision expenses, including copayments and deductibles. See a list of generally permitted medical, dental, and vision expenses.
  • FSAs are available only with job-based health plans. 
  • You can’t spend FSA funds on insurance premiums.
  • Visit the FSA store to find an extensive list of eligible products. When you use code CBHRP5, you can save $5.00.
